Merge branch 'jk/diffcore-rename-duplicate'

A corrupt input to "git diff -M" can cause us to segfault.

* jk/diffcore-rename-duplicate:
  diffcore-rename: avoid processing duplicate destinations
  diffcore-rename: split locate_rename_dst into two functions
