vm: Factorize input conversion.

* gnu/system/vm.scm (input->name+output): New procedure.
  (expression->derivation-in-linux-vm): Use it for 'input-alist'.
  (qemu-image)[input->name+derivation]: Remove.  Use
  'input->name+output' instead.
